Your Discharge
When the doctor decides you are ready to leave the medical center, a discharge order will be written. You may want to make arrangements with a family member or friend to transport you when it is time to go home.

Discharge Instructions
Your doctor and nurse will give you instructions about your care after you leave the medical center. If you have any questions about your medications, diet, activities, or other matters, please be ask your doctor, nurse or discharge planner.

Personal Belongings
Collect all of your belongings and double-check closets and drawers. If you have valuables stored in the medical center safe, please contact a member of the health care team for assistance in retrieving those items.

Escort Service
When you are ready to leave, a member of the medical center staff will escort you to the main lobby, unless prior evaluation allows you to leave unaccompanied. The nursing staff can call for transportation as needed.

Medications
Your doctor may prescribe medications for you upon discharge. It is important that you inform your health care team of any allergies or reactions you may have had to any medications in the past. The staff will give you information about your medications.

After You Leave the Medical Center
There are occasions when patients need additional treatment or care after they have been discharged from the hospital. To meet the special needs of these patients, a member of the Cumberland Medical Center staff will be happy to further assist you.
