| “What
is case management?”
Case Management is an approach used to plan your health care. Case
Management does not replace the contract you have with your physician.
It requires the teamwork of many healthcare professionals, but it
always centers on the patient.
The Case Management Department identifies those patients
for whom discharge planning is critical.
This discharge planning process is initiated early in the treatment
process based on requirements of the plan of care. The discharge
planning process is individualized and may vary based on age and
disability considerations and treatment objectives.
You will be informed in a timely manner of the care process and
updated in the event transfer or discharge to another level of care
is anticipated.
