“What is case management?”
Case Management is an approach used to plan your health care. Case Management does not replace the contract you have with your physician. It requires the teamwork of many healthcare professionals, but it always centers on the patient.

The Case Management Department identifies those patients for whom discharge planning is critical.

This discharge planning process is initiated early in the treatment process based on requirements of the plan of care. The discharge planning process is individualized and may vary based on age and disability considerations and treatment objectives.

You will be informed in a timely manner of the care process and updated in the event transfer or discharge to another level of care is anticipated.

"What Do the Case Manager and/or Social Workers do?”
The Case Manager and Social Workers are professionally trained members of the healthcare team who assist patients and families in many ways during hospitalization and during the discharge planning process.

“What can Case Management do for you?”

  • Help you and your family deal with your illness, rehab, and accompanying stresses.
  • Serve as a liaison between you and your family, the doctors, nurses and other members of the healthcare team.
  • Help you and your family with financial aspects of your hospital stay.
  • Help you locate and coordinate available resources and community agencies.
  • Work with you and other members of the healthcare team to coordinate your post-hospital care.
