Diabetes Self-Management Program - Woodmere Mall, Crossville

The Diabetes Self-Management Program is a service of Cumberland Medical Center. The goal of the program is to provide an organized multi-disciplinary approach to diabetes self-management for patients and their families. The diabetes program has achieved American Diabetes Association Recognition for meeting the National Standard for diabetes self-management education.

Program Overview
The program utilizes a team approach which will be developed and individualized for each participant in collaboration with their physician, a nurse educator, and a registered dietitian. Using the Life Skills Diabetes Education Program, the following areas will be presented: A diabetes overview; meal planning; blood sugar monitoring; sick day management; exercise; medications; prevention of complications; stress management, and more.

Program Entry
After referral from a physician, diabetes staff will contact the candidate and schedule their appointment. The program, presented at the Wellness Complex in Crossville, consists of a one-hour initial visit followed by 9 hours of group sessions, and can be completed in 4-6 weeks. During the program, the program’s staff works closely with the patient’s physician to provide care including updates and reports of patient’s progress.

Professional Staff
The Diabetes Self-Management staff includes a physician medical director; registered nurse; registered dietitian; certified diabetes educators; an exercise specialist; and program secretary.
